The physician must complete and then fax the Prescription & Enrollment Form to Walgreens at (888) 347-3416.Once the completed enrollment form is received and processed the patient (or caregiver) will be contacted to schedule shipment of the drug.

VA/FSS Customers Only: There are two options available to order Matulane:

(1) Order through Walgreens Specialty Pharmacy and ship Matulane directly to the patient’s home or to the VA Pharmacy for the patient. The physician must complete the Matulane Patient Prescription Form- Veterans Administration (VA only) and then fax the form to Walgreens at (888) 347-3416.

(2) Order Wholesale to ship to facility through Integrated Commercialization Solutions (ICS), on behalf of Walgreen Co., by completing the VA/FSS Wholesale Ordering Authorization Form and then fax to ICS at (855) 534-8308 or call ICS at (877) 534-9627.
